Short Break Service
Once a referral has been made, we will apply to the referred person’s GP for a written diagnosis of dementia. When this has been received an assessment will be carried out within the home environment to ascertain the level of care required and suitability.

Our out of hours service operates outwith the hours above. Out of these hours we have a dedicated 24/7 on call service which is automatically directed to the on call Manager via our main office number.
