Palliative & End of Life Care

End of Life Care offering respect, compassion and dignity.

Line break

Our home care staff ensure that the care people receive at the end of life is compassionate, appropriate and gives people choices about where they spend their final days and how they are cared for. In partnership with the Prince & Princess of Wale hospice, our staff are trained in End of Life care. This training ensures they are prepared and able to make a positive contribution when it is needed.

The principles and competencies that form a common foundation for our specially trained staff encompass communication skills. Good communication skills are vital in ensuring the needs of people nearing end of life, their families and carers are identified, responded to and met – with respect, compassion and dignity.

Our staff are taught to undertake and contribute to multi-disciplinary team assessments and share information with other care and health practitioners, ensuring that all assessments are holistic.

Our care staff and care managers are fully trained on Advance Care Planning guidelines for End of Life care. We ensure that your plan is communicated and respected at all times.
